Standard anesthesia, radiology, and pathology fees included in the above prices. Standard dietary pre-surgical visits and psychiatry evaluation are included.
Additional services or complex surgeries may involve additional fees not listed above. Some procedures may require additional medical providers as an extra cost. Your bariatric coordinator will explain any additional fees.
All fees per applicable fee schedules must be paid to hospital/physician at least seven (7) days prior to services being performed or services will be cancelled or rescheduled.
